An opportunity has arisen for enthusiastic, innovative and forward thinking Registered Nurses (Adult) - all disciplines, to join the Manchester NHS Continuing Healthcare Service & help deliver our new vision for NHS Continuing Healthcare across the health & care system in Manchester.

You will liaise with patients, their relatives and other healthcare professionals in a range of settings to assess patients’ clinical needs and be skilled at effective case management to ensure those patients’ on-going needs are met within appropriate settings and placements.

As part of a large team of CHC Nurse Assessors and Nurse Managers you will ensure that reviews are carried out in a timely way, using eligibility decisions for NHS Continuing Healthcare in line with the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.

You will be working in collaboration with colleagues across the health and social care system in Manchester and other areas so that improved outcomes for patients are understood & prioritised. The maintenance of our e-case management system will be an integral part of the role to ensure work is undertaken in accordance with the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.

As Continuing Healthcare assessments and reviews can be carried out in individuals own homes, nursing homes or other alternative care settings, independent travel across the City of Manchester is essential.

You will work under the guidance and support of the senior clinical management team promoting best practice and providing professional advice to nursing homes and other providers, liaising with colleagues across the health and care system to promote positive relationships

To succeed you will be an experienced first level registered nurse (Adult), with a detailed knowledge of relevant legislation, be able demonstrate excellent interpersonal, negotiation and communication skills, who thrives in a very busy, team focused, performance and quality driven, professional and rewarding environment.

To undertake this role, the successful applicant will be required to provide evidence that they are fully vaccinated for Covid - 19 as mandated by the Health & Social Care Act amendments July 2021.

Greater Manchester Integrated Care Board (ICB)

In February 2021 the Government announced a number of new measures as part of a proposed Health and Care Bill, including the creation of statutory Integrated Care Systems (ICSs).

In Greater Manchester we have been working as an ICS for the last five years – with strong working partnerships between health and social care and the voluntary sector. The creation of a statutory ICB will formalise the arrangements we have in place and mark another positive step in our journey, which started with the health and care devolution deal in 2015. The statutory ICB will include a new ICS NHS Body and Health and Care Partnership Board.

From April 2022 (subject to national legislative approval) the commissioning duties of Clinical Commissioning Groups (CCGs), including their functions and roles, will transfer to be delivered as part of the new Greater Manchester ICB NHS Body. This will mean that functions and roles may be delivered differently in the future, with place based and Greater Manchester-level teams working alongside each other.
