PCN Lead Pharmacist

The Medicines Optimisation Team has a long-standing history of delivering high quality services to support the work of General Practice across the City. The successful candidates will be welcomed into the expanding medicine optimisation team with the unique opportunity to develop clinical and leadership skills within MHCC, PCNs and practices.

The practice based medicines optimisation team at MHCC were formed three years ago as a service provided to North Manchester. Following the success of this service, the team are now embedded in PCNs across Manchester, supporting PCNs, practices and the local population to improve health outcomes.

Prospective candidates should be able to demonstrate the ability to lead and influence, and promote the role of the pharmacist and their expertise in medicines and prescribing quality across the health and social care system. Candidates will also be expected to have an in-depth knowledge of Primary Care Networks and the role of the medicines optimisation team within the networks.

The post holder will be expected to:

Provide expert pharmaceutical, prescribing and medicines management advice to other healthcare professionals, GP practice staff, care homes and community services teams and acute hospitals
Provide leadership to PCNs, PCN medicines optimisation teams and work closely strategic medicines optimisation team
Demonstrate a sound understanding of the health economy, primary care and the changes to health and social care within Greater Manchester
Demonstrate leadership qualities; promoting the role of medicines optimisation in health and social care, act as a role model for the medicines optimisation team and be able to influence and negotiate practice to improve outcomes for patients and service users
Commit and contribute to the financial stability of local and wider health care systems through promotion of high quality, effective prescribing and avoiding medicines waste
Be adaptive and flexible; open to contributing to a variety of workstreams and projects within working capacity
Work autonomously, but recognise their limitations and seek out and accept support of the wider team to overcome these
All candidates must hold a pharmacy degree and be registered to practice with the General Pharmaceutical Council, hold a postgraduate Clinical Pharmacy Diploma or relevant alternative experience/qualifications. Applicants must be able to have the means to travel the Manchester footprint. Toundertake this role, the successful applicant will be required to provide evidence that they are fully vaccinated for Covid – 19 as mandated by the Health & Social Care Act amendments July 2021.
